Add name back into fieldRef and componentRef elements

Would like to add back in as an optional field the ability to include the name of the field or the name of the component in addition to the id of the component on the fieldRef element and the componentRef element. This saves a look up in some instances and also makes the XML a bit more readable if someone is looking at raw XML. Tablature already supports ID and Name in its format.
