Option to use setext headings

Since setext headings only support <h1> and <h2>,
the problem is what should HTMLarkdown do when it encounters a <h3>Heading 3</h3> when in setext mode?

Should it:

  • switch to ATX-style? (ie. ### Heading 3)
  • or switch to HTML-syntax? (<h3>Heading 3</h3> )
