Add Ignore Button w/ Explanation

Help cut down on users frivolously ignoring alerts by requiring an explanation with ignore.

Maybe the explanation can have a "false positive" option that would then be able to report to axe or other scans we're using?

I think allowing an option to explain an ignore would be terrific! But I don't think it should be required. I think that people can be trusted and police themselves with which issues to ignore.

Right @joelhsmith. Required should be something we move into if people abuse the ignore button. Just having the extra step might be enough of a deterrent from ignoring every issue, like we saw with older versions of Equalify.
