Remove Internal Iterator

remove functions related to internal iterator:

• ksRewind
• ksNext
• ksCurrent
• ksGetCursor
• ksSetCursor
• ksHead
• ksTail

change ksAtCursor -> ksAt document new way of iterating

This list should also include the versions for the metadata keyset, e.g. keyNextMeta and keyCurrentMeta.
