Hey there! Have you found a solution for the bug?
Sadly, no. I just assumed my PC is just too outdated and won't be able to run shaders properly anymore and I moved on.
Try my mod setup and see if the issue persist (You may ignore Xaero's Minimap)
It is most likely cause by one of the optimization mods in the modpack.
I already tried that before.
The culprits that could cause this glitch may be from More Culling, Enhanced Block Entities, and Entity culling from the modpack you used. Try removing those and replace it with Immediately Fast.
If anything, do not use the modpack that you're using if you haven't removed it when I asked you to try my mod setup. Many optimization modpacks tend to have a lot underlooked issues.
I followed everything you told me, and the issue still persists.
If you have reloaded your mods several times and even check logs if all the mods have been loaded correctly then I honestly have zero clue why this happens. You have not changed your Sodium settings or your shader settings right?
I have not touched the shader settings at all, the only Sodium settings I touched were render distances. Could that be it?
I don't think that's very much possible that render distance was the cause of it. Are you using another launcher? Determining from this picture with a unique UI.
Could you also confirm that this issue does not occur with other shader and send me your hardware and OS specs?

That's the Prism Launcher, a fork of a fork of MultiMC.
Could you test if this issue occurs also in Optifine? If not, please report to the Iris team.
I'm deeply sorry for the very late responses, I honestly just keep forgetting about this. My bad.
Anyways, I've tried it with Optifine and upon enabling the shader my game instantly crashes. Here's the log file: https://mclo.gs/twXImgi It's a clean Forge install of the game with the only mod installed being Optifne. I honestly think this issue is on my end due to having too outdated hardware. (OpenGL issues, I guess.)
Closing this was not intentional.
Could you try loading with Optifine instead of loading Optifine through the Forge loader? Also, the file you gave me is not clear, sending a fresh (blank) latest.log or enabling Minecraft's output log before launch and monitor the log live.
Issue still persists.
Are you saying the entities are still broken with only Optifine (not Forge loader) just as the same with Iris? Please answer clearly.
With Optifine it just straight up crashes, with Iris it loads but is broken.
What do you mean Optifine (not Forge loader) straight up crashes? As in the initial startup before loading the shader? Again, please be more specific and provide a fresh latest.log file.
Upon enabling the shader with Optifine installed my game instantly crashes.
And the latest.log file...?
