Issue Description

As described in a lost note at the bottom of #584 , in clubb_intr currently out_ clubb history vars are allocated based on the stats_zt(1)%num_output_fields field. It would be better if they were allocated based on a field that does not change per column and doesn't require a hard-coded reference to the first column of stats_zt.

The CCPP-ization of CLUBB (see #1263 ) will likely fully refactor the history output from clubb_intr.F90 and address this issue. I'll put a note over there as well.
