TJ16 incorrect for MPAS

What happened?

etamid is not initialized (and is not well defined) for height coordinate models (e.g. MPAS). Remove etamid depedency and use actual pressure instead in TJ16. (likely grey radiation setup has this issue as well)

Run TJ16 with MPAS

What machine were you running CAM on?

CISL machine (e.g. cheyenne)

Intel
