Misattribution fixes

Fixes most of #1997 ~~and maybe some of #1144~~. Fixes #2692.

  • [ ] Review Badger Sett scans (should render cleanup unnecessary)
  • [ ] Assess performance impact of updating performance-critical request blocking code paths

Should be followed up by #2024.

Should address Firefox Bug 1484647: uMatrix and Privacy Badger do weird things.

Update: Fixed by #2454.

Should be able to remove cleanup from badger-sett after this gets merged and we have a few badger-sett runs to confirm that's the case.
