Update Congress Forms markup for redesign

Once we land the congress forms refactor, we should merge master into the redesign branch and update the congress forms templates to reflect the redesign. Required changes include

  • No em wrapping "common fields" and legislator names
  • Toggle submitting to each legislator using a checkbox next to that legislator's name
