Add column to associations download to distinguish OR from beta

Several users including Ensembl have asked for an easier way to distinguish OR and beta in the associations download. We should add a column to the associations download called "Effect size type" which states OR or beta.

This could be done at the same time as adding a column for sumstats in the studies file, and improving the v3 downloads.
