cdc disease and conditions A-Z sublist xrefs

There's interest in adding cdc xrefs to wikidata per Gene Wiki Central Issue #39. Greg's initial investigation on this issue indicated that there were 77 DO xrefs with CDC, which seemed small so I scraped the Center for Disease Control's Diseases (CDC) and Conditions A-Z sublist and mapped the urls indexed in the sublist to WD entities and subsequently DOIDs. The mappings are listed here, please add the cdc xrefs to DOID.

Since CDC is not a clinical vocabulary, I would add these to DO as reference refs, but not concept xrefs. Since the disease term xrefs only link to other clinical vocabularies.
