Failed to teleport, crashing server, and other errors

The title is self-explanatory. Below will be all the logs, information, etc. Something I also want to see fixed is when teleporting to limbo, sometimes, I get put outside the world border. Sometimes, I'm even put 153k blocks or any absurd number away from 0, 0. Since people cannot place blocks outside the world border, they are basically stuck there
