implement indexed variants of `ap` and combinators

Current Behavior

I see that there is no iap, which would be nicer than using ichain for chaing the status type.

Desired Behavior

API additions:

• iap
• iapFirst
• iapSecond
• iapFirstW
• iapSecondW
