This is a default screen when connecting a Trezor if you have a passphrase enabled. There is no "creation" of hidden wallet, passphrase is simply said another seed word added...

It's totally unreliable, sometimes it uploads the file fine, sometimes with zero size, sometimes multiple times, sometimes just once...
Since 4.0 beta does have the problem as well, I would be surprised if it was fixed in 3.0.4, but maybe some accidental change fixed it, let us know if...
The original problem seems to be fixed now, at least with 4.0-beta4 I didn't notice duplicate files anymore.
