Player may sometimes not get a quest on defending the camp being attacked

I may be wrong, but when playing vanilla I assumed it's related to the fact if the enemy squad saw me or not. Because when it saw me and got in combat, I never got a quest on defending the camp from that squad later. I may be wrong though.
