Weapon sounds (reloads, foley etc.) often do not play while indoors
This is a longstanding RO bug, and it quite an annoyance once you notice it.
You can get it to happen pretty easily but just walking into any house (as long as it's made of BSP), and reload your weapon. You will very often not hear the reload sound, or it will be INCREDIBLY faint, as though it's being played far, far away.
I found out the root cause of this. It turns out it's a bug with animation notifies being used to play the sounds. The specific notification type it uses is incorrect. The fix is to go through literally every DH 1st person animation file and change the notification type.
Was looking around and noticed many using the correct sound notify, not sure what the wrong one is?
