repeatedly prompts with "CuisVM.app" would like to access files in your Documents folder

I'm running on a Mac. Every time this dialog appears I click "Allow". But the dialog reappears any time I select "Do it" for some code. When I open the System Settings app, I see what is in the attached screenshot which indicates that I have granted access to everything.
