Tracking Issue: Changelog approach

As we get more formal and disciplined about using the EIP lifecycle and iterating CAIPs in "Review" mode, we should add changelogs any time a substantial normative change is made to an accepted/Draft CAIP.

at yesterday's editorial meeting, we agreed to add a changelog manually with a dated entry per major/breaking change when CAIPs go from Draft to Review and use them to monitor staleness.
