Instructions for migrating from 0.6 to 1.0

On several occasions, people have asked in the forum thread for instructions for migrating from 0.6 to 1.0. I don't blame them for not searching through nearly 200 pages to find my post where I share my technique, but they might be headed off if some instructions were included in the readme file (or in a standalone "readme_migrating" file, as the most recent questioner asked).
