Use Case Title Add Moderate to Incident Injury Type and Concatenate Serious and Major

Use Case Description The element "Incident Injury Type" contains two options that are very similar in nature: Serious Bodily Injury and Major Injury. The recommendation is to remove Major Injury and keep Serious Bodily Injury. Additionally recommend adding in a middle tier "moderate injury" to fall between Serious Bodily Injury and Minor Injury.

Use Case Background No definitions presently exist for the options in Incident Injury Type making it difficult to differentiate between Serious and Major. Recommend also adding definitions into the element options.
