Add missing telecom use code for phones in XTN-2

Why it needs to be done

APHL requested this be populated

Acceptance Criteria

• Use PRN as Personal Residence Number for patient phones (most applicable option available in HL7 2.5.1 from table 0201 Telecommunication Use Code)
• Use WPN as Work Number for provider and facility phones
