2.0 Form: Allow address unknown on reporting form

User Story

As a 2.0 form submitter, I want to be able to check a box Address unknown, so that I can submit reports for unhoused patients or patients who otherwise do not have an address.

Acceptance Criteria

• A new checkbox is added to the form Address unknown or patient unhoused
• When checked, the address fields are hidden (similar to the behavior on the 1.0 patient form)
• When checked, hidden field validation does not trigger
• When unchecked, the fields reappear and their validation behaves as usual

Scenarios

For example, this could look like:

• I fill out the patient information, leaving address fields blank
• I click on the Address unknown or patient unhoused checkbox, address fields disappear
• I am able to hit the next button and submit the form
