[wip] Map AOE & Device data to HL7v2 for legacy messages

What needs to be done

• Still TODO on requirements side: figure out where in the spec this data should go. That might turn into a spike for eng.
• Map our AOE questions and device data to our HL7v2 message

Why it needs to be done

Parity with existing behavior of the legacy app.

Acceptance Criteria

• [ ] All AOE questions that are currently mapped in fhir are mapped in HL7
• [ ] Device data is mapped in HL7
