Error - "New Polygon" Function Ignores Point(s) During Polygon Creation

To recreate - (1) click "New Polygon", (2) click on World View screen to create a polygon vertex, (3) continue step 2. until the polygon rendering ignores one of the vertices. See screenshot. (4) Double click on the same point to close the polygon creation mode, and a blue polygon is shown, which ignores the ignored points during the polygon editing mode.
It does not always happen. Any idea what causes it?
When creating a polygon, at least one vertex-not-on-polygon instance always occurred. I repeated many times to try and identify a patten but I couldn't. However, it doesn't seem random. After re-starting the toolbox and creating a new polygon, it (polygon not taking account of vertex) seems to happen on the nth vertex (where n is about 4th or 5th), and then when I create a new polygon it seems to happen on the n-1 vertex. But maybe this was chance, because of the small sample size.
I'll debug into it.
