Bug report function under Help

Create a simple "interview" style dialog under the Help menu to enable users to submit formatted bug reports more easily. This could prompt for various questions (ie., "what was the modality", "what application were you running", etc.) and would produce a formatted text file, then display instructions for the user to upload (drag & drop) into Github.
