That version only works when installed via homebrew, as if requires homebrew package for highlight. https://github.com/caskroom/homebrew-cask/blob/master/Casks/qlcolorcode.rb

what was the full URL of page you were visiting, it should appear for any url containing http/https

3.x install is currently failing. Stable should work

Just install again and it asks if you want to uninstall.
