XREFs to footnotes/references do not current render in headers.

Is your feature request related to a problem? Please describe. Currently if you add a link to a footnote or reference in a title it will not display the link element.

Describe the solution you'd like As well as tagging it as <sup> the link element to the footnote/reference should render.
