Not sure I understand what you're asking here. Did you read https://www.bionus.org/imgbrd-grabber/docs/duplicate-files.html#the-md5-list ?

If you want to always download images, even if they're already downloaded somewhere else, you can configure it like this. If it's only about moved/deleted files, you can change that setting as well.
