Module Issues With Teacher's Name

We have multiple long outstanding open issues related to modules. Most seem to stem from how Sensei appends the teacher's name to a module.

It's likely implemented this way because modules can be shared between courses, but teachers can only access their own modules. If we move a course from one teacher, those modules can't just switch ownership to the new teacher because the old module might still be used by the old teacher in another course.

We need to triage all possible related issues and work on a plan for improving this behavior.
