Refactor and improve proxy based tests

Since 48111becfa6b2cd19cbc2ead9f72ae976e340bd0, we can build tests that really use proxies, not just mocks.

We should build on them, refactor the existing tests, and add some more such as preemptive-vs non-preemptive auth, proxy auth + server auth combo, etc.
