template example missing module.js file

The provided template example in the wiki is missing a javascript file (module.js)

The example code should also be restored to the main repository (like it was in older versions, though perhaps in an 'examples' subdirectory).

It looks like sfranzyshen put a lot of work into refactoring things for 4.0, but without updated examples, its nearly impossible to migrate. I'm seeing a number of odd issues, but don't know what's user-error versus potential issues in the library. For example, in my initial experiments, 'round-right' doesn't show up consistently for buttons, which in turn won't show up at all f I omit another element.

I pushed my initial experiments to a fork at https://github.com/Digicrat/LCARS-SDK/tree/demo The module.js file is commented with the issues I've noticed so far, though I haven't spent much time looking at it yet either.

(Fyi, I started using this framework for my home automation system a few years ago ... and have finally come back around to resurrecting that initial UI concept. I've yet to find another library/theme that provides as satisfying an interface for a touchscreen control panel).
