Create a new `create` page

Although the app is pretty good at explaining what it does, it can be difficult still for people to explain exactly what's going on. Also, auto-generating and redirect setup (although this is all client-side) could be jarring for some.

It'll be handy to have a 'create' page where it's sole purpose is to explain things a bit and create and extra-step to generate a report.
