Fire Temple bugged?

I am completely confused here and think my temple might be broken. I have finished the Fire Temple entirely, I entered every single room, looted every chest, freed every Goron and killed Volvagia, and yes I looted the heart container after. The item menu pop up tip says that my final missing Medallion (Light) comes from this temple. Here is an image album showing every room has been entered, no chests remaining and the light medallion marked on my hint pop up.
