Error when creating new/second user: Request failed with status code 422

• Have you tried using the latest docker image / code base : yes

Describe the bug When I go to create a new user and fill in the user details I get a red banner error 422 when pressing 'save'

Steps to reproduce the behavior:

1. Go to Admin -> Users -> Add User
2. Fill out user details
3. Click on save
4. See error Request failed with status code 422

Expected behavior A new user is created and added to the user list
