Clean up CALM example

The current CALM example has unused fields in the input data files. These fields can be confused as mandatory fields required to run PopulationSim. The input data files should include only the required fields.

See issue #90 - I suggest adding a more recent example, and removing ODOT's original CALM example. This should be a low level effort that can be resolved in Phase 9.
