prune "where is extension" section, update README.md

-removed "where is extension x" section as it is now installable through the ui. -gfpgan instructions not needed, it will auto-install

-blend upscalers (a user pointed out such a thing was possible, which i didn't know https://github.com/AUTOMATIC1111/stable-diffusion-webui/discussions/3749#discussioncomment-3978277)
