Metadata form refactoring

Refactor the Metadata form to reduce cognitive load:

  • [ ] Remove the Image / Video button. The tool should handle this without needing the user to choose
  • [ ] Use the new form component for url and file upload
  • [ ] Use a "tip" for the additional text in the form
