Add exception codes

Doing explicit exception handling is good, but leaves client applications to depend on the messages the API actually returns.

Proposed Enhancement: Add an exception code and document the exceptions which may occur. If clients are not happy with the exceptions the API returns, they can map from the defined exception code to own exception messages and add localization (aka i18n).
