[Bug] Maxkb dialogue model context is stuck, and the token input length is accumulated, resulting in the initiation request failure

Problem Description

多次上传大文本给AI助手，上下文内容莫名累加文本长度导致无法请求，新建对话也累加。ai模型不可用（已发布应用不影响，新建应用受影响）。

• 重启maxkb服务和数据库服务均不可恢复
• 同模型重新添加恢复正常，原模型仍不可用

Steps to Reproduce

无法重现，但测试环境和日志保留，卡住的模型还未删除。
