**From Liz's Email ** The request is to add a new upload option on the Wage Data screen, specific to when a Prevailing Wage Survey is selected from the question, “Which method did the employer use to determine the prevailing wage for the job or contract identified above?”

Requirements:

• [ ] One document upload should be available within the prevailing wage survey box (location indicated below by red star – either below the Add Source Employer button or below the grid of Source Employers dependent on view)
• [ ] Label by upload should be, “Attach an additional sheet headed “Prevailing Wage Determination—Hourly” if needed.” or “Attach an additional sheet headed "Prevailing Wage Determination—Piece Rate" if needed.” (dependent on whether within hourly or piece rate section)
• [ ] Upload is optional
• [ ] If prevailing wage survey is selected, all required fields remain the same and upload is optional
