https://github.com/koppor/adr-manager could be one possible UI. It is going to be submitted to the IEEE Score Contest. Thus, a first release is scheduled for January. In case the submission is accepted, there will be a "final" version in May 2021.

The simplest prototype for this is to reuse github templates and provide a link to create a new issue.
