Fix Format SQL bundle outdated

Since http://www.dpriver.com/ has improved their site so the Format SQL has been outdated and always show error message Please refresh this page to load the latest version 4.x. I made some modifications here for this bundle to work properly again.

Can't merge this.

  • See my comment in https://github.com/sequelpro/sequelpro/issues/1988#issuecomment-144577691 regarding the owner's wishes
  • There is a lot of pure white space change in that commit. That would need to be removed.
