Cleanup PR/issue template and changelog generator script

- Let the author choose kind/bug or kind/feature from doc issue template.
- Update the PR template
- Update the changelog script to take into account the new changes.

/hold I think it's best if we merge this after the release of v3.1.0 so as to avoid missing out on issues due to label selection change.
