@wuzhen97 which sections has this occurred to you on (page header, titlebar, status bar, etc)? I have noticed this occasionally as well and will look into how to replicate.

titlebar occured
@jsmorabito . It happened again today, and it is always the one, which command is provided by the obsidian-workspaces-plus plug-in.
I hope this information is helpful to you.
Same, but I tried to add a button on the status bar. It seems that the obsidian-workspaces-plus plug-in causes this issue.
I keep having my status bar commands disappear. The commands I put in the status bar are for the Status Bar Pomodoro Timer plugin. It turns out that the Status Bar Pomodoro Timer plugin is not supported on IOS (iphone or ipad). I'm wondering if it's an issue with the commander data.json file on my PC getting synced with a version from my iphone/ipad and Commander eliminated the commands in the IOS version because they were not available on my ipad. Does Commander eliminate commands in the data.json file where it thinks the plugin doesn't exist?
I just noticed someone said that they were having a similar problem with workspace plus. Are they syncing with a device that doesn't support the plugin. I know that plugin is not supported on IOS
