Write some initial API documentation

At minimum, I'd like to write function comments for everything important, but we should probably have a slightly-better docs page too, e.g. including docs about NODE_ENV.

• [ ] write docs in code (npm run doc should catch them and create api.md)
• [ ] add API guide to mkdocs.yml so when built its in the navigation
