Add Changelog for SQL Database Projects extension

Would be great if you could add a CHANGELOG.md to your project so it is picked up by VSCode and also shown in the VSCode marketplace

@gbrueckl could you please clarify which project his is referring to? Azure Data Studio's changelog is at https://github.com/microsoft/azuredatastudio/blob/main/CHANGELOG.md. But given the reference to VS Code, maybe you are referring to the SQL Projects extension?

this was for the SQL Database Projects extension: https://marketplace.visualstudio.com/items/?itemName=ms-mssql.sql-database-projects-vscode. Somehow I got redirect here I guess
