Add confirmation prompt to avoid accidental removal of an index

In the plugin page, clicking on any row of the index will uncheck the index and remove it.

It is very easy to lose all the manual index settings just by an accidental click. Adding a confirmation prompt will avoid such situations.

You now have to explicitely click on the checkbox since v0.4.0. Does this solve your issue?
