Add Nightscout Blood Sugar plugin

This plugin shows graph and trend data for Blood Glucose monitors via the Nightscout system: https://nightscout.github.io/

Realised there is already an existing Nightscout plugin but it is text only, updated plugin name to reflect the difference :+1:

The graph looks amazing! I'll try to add it to the "LibreLinkUp Status" plugin (I don't have nightscout setted up). Thanks for the contribution, let's hope it gets merged soon :D
