Add `CHANGELOG.md`

Currently there doesn't seem to be a changelog in this repo.

This makes looking for changes or even knowing what's new in a release quite difficult.

There is also no GitHub release associated to the published tags.

So adding a changelog would greatly help users and consumers of xeus-python.
