bug: disabled mcp tool but still being invoked by model

Describe the Bug

I've disabled web search tool from chat input tool list, but it still getting called by the model. See attached screenshot

Steps to Reproduce

1. Create a new chat
2. Disable web_search tool
3. Ask model to search
4. It still use web_search tool
